Regional Complex Care Coordinator
We are seeking a highly skilled and dedicated Regional Complex Care Coordinator to join our team. This is a full-time position based in North, North West Region.
The Child and Youth Mental Health Service in Tasmania requires a passionate and experienced clinician to provide high level clinical coordination services for children, young people and their families/carers who present with complex mental health concerns.
Key Responsibilities
* Providing specialist clinical coordination service that supports right place, right care principles to facilitate determined need.
* Undertaking high level assessments and developing an individual plan that includes bringing together the right mix of services across government and non-government agencies.
* Working within a well-supported, diverse multidisciplinary team to provide clinical co-ordination, assessment and developmentally appropriate therapeutic interventions.
Required Skills and Qualifications
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
* Possess specialist tertiary graduate or postgraduate mental health/psychiatric nursing qualification.

This position will work alongside existing CYMHS services and have been developed as initiatives under the CYMHS reform program (Youth, Youth Forensic, Out of Home Care).
The role will include leadership training provided by the Tasmanian Health Service in partnership with University of Tasmania.
