This is an opportunity to work within the Enhanced Care Program, coordinating activities that promote emotional wellbeing and social connection for patients. The coordinator works under the direction and supervision of the Registered Nurse to deliver high-quality care.
Key Responsibilities
* Develop and implement activity programs that meet individual patient needs
* Work collaboratively with healthcare teams to provide person-centred care
* Support patients to engage in activities that promote emotional wellbeing and social connection
* Maintain accurate records and reports as required
