Job Description:
As a Care Coordinator, you will play a vital role in enhancing the delivery of our aged care services in Hervey Bay and surrounding areas. Your responsibilities will include processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs.
You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
In addition, you will contribute to program coordination by assessing, improving, and overseeing operational and administrative systems. You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering the growth of our aged care services.
This is a remote working opportunity with some travel required to visit clients in their homes, including to Bundaberg and Maryborough.
