Tasks
* Provides direct clinical care including personal care, as directed by the Facility Manager
* Undertakes initial and ongoing assessment and interpretation of care recipients within the framework of holistic professional nursing practice, and in accordance with the principles of person centred care.
* Assesses and interprets information and data regarding the care recipients' culture and values system, lifestyle, and interest and ability to participate in their care, and other factors impacting successful treatment outcomes.
* Provides ongoing monitoring of care recipient and/or representative response to intervention; identifies deviations from expected outcomes and assesses change in care recipient status and responds in accordance with best practice guidelines.
* Reviews and updates the care recipients individualised care plan, in collaboration with the care recipient and their representative, the multidisciplinary health care team and other providers of care promoting continuity of care across the continuum.
* Identifies, initiates and coordinates therapeutic interventions consistent with the legislated scope of practice and level of competence.
* Implements prescribed interventions (e.g. diagnostic tests, medications) and performs independent nursing interventions (e.g. care recipient and representative education).
* Ensures the implementation, coordination and ongoing evaluation of the care recipient care plan.
* Ensure care staff complete care recipient's observations including weights as per policy and procedures and ensure that all reporting is completed for a care recipient as appropriate.
* Identify and manage care recipient wound care requirements and ensure associated documentation is completed as per the policy.
* Ensure 'clinical' rounds are attended during the day, at the allocated times to ensure services are delivered in accordance with the expectations.
* Ensure all clinical changes are reported during hand over and to the Facility Manager.
Identified and becomes responsible for a clinical theme, assisting the Facility Manager with compliance.
Essential criteria:
* Comprehensive understanding of the Aged Care Accreditation Standards and recent changes or a background in caring for older people in a residential setting
* Possess a compassionate, caring and supportive nature to provide quality care to our residents
* Strong leadership qualities
* Excellent verbal and written communication skills
* Excellent clinical documentation practice
* Excellent time management and organisational skill
* Be committed to ongoing clinical excellence to providing the highest standard of care & support
* Display a relationship-centred approach to care and supervision
* Great understanding of care planning and evaluation documentation
* Current AHPRA registration
* Current police check or willingness to obtain
* NDIS worker check or willingness to obtain
