North and West Remote Health (NWRH) is currently seeking an adaptable Journey Coordinator within the Stepped Care service system to join our team.
The Journey Coordinator is a dedicated position that operates as an independent partner in a person's care journey. The role may be delivered as a standalone service, within a Front Door service, or as an adjunct to another Stepped Care service offering. The Coordinator will place individuals at the centre of their own health and wellbeing and help them achieve improved outcomes.
Responsibilities
Your primary responsibility will be to deliver high‑quality support and services by improving outcomes for Stepped Care participants. You will build psycho‑social education, support access to self‑management approaches, including digital mental health services and group programs, and facilitate the Stepped Care model to ensure the quality, continuity and coordination of care between service providers.
This role is offered on a part‑time basis in Mackay and involves outreach travel.
About you
* Ability to establish, develop and maintain multiple productive working relationships and networks.
* Experience identifying and delivering partnership and collaboration opportunities that enhance consumer outcomes and improve the service system.
* Understanding of mental health challenges and how they inform an individual's experience.
* Ability to develop and maintain a genuine relationship with individuals accessing Stepped Care.
* Recovery‑focused, person‑centred, strengths‑based and trauma‑informed approach.
* At least a Certificate IV in a mental health related qualification. Only those with suitable qualifications will be considered.
