It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991.*****About Us?**The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.Staying Deadly MOB team is an outreach service providing outcome focused health, social and practical support to Aboriginal and Torres Strait Islander clients. The Staying Deadly MOB team walks alongside to provide support and access to services which guides mobs social, emotional and cultural needs through hearing, supporting and empowering mob.**About the Role?**The role focuses on developing personalised care plans for clients, hearing their story, identifying their needs and empowering client to make their own choices. Working within the wider IUIH system-care to effectively connect client with clinical and non-clinical professionals to meet their wellbeing needs. As a Care Coordinators, you will be walking with clients who may be struggling with homelessness, mental health issues, AOD use issues, chronic disease and/or transitioning out of corrections centres into community by providing social support and linkages through intensive case management and referrals.**Your day-to-day duties will include:*** Participate in care coordination activities to fulfil needs of assigned clients transition care plans, including coordination meetings with relevant external stakeholders* Working flexibly in response to client and family/carer needs* Conduct home, community and/ or custody visits to support ongoing client recovery* Liaising and developing external relationships with services/agencies to address client needs* Contributing to smooth transitions for client who are accessing other services* Keeping accurate and confidential, current case notes, statistics and other client records and files* Developing networks and building community support for promotion and participation in services**About You?*** Current C Class Drivers Licence (QLD)* Current Criminal History Check* Valid Blue Card* Current CPR and First Aid Certificate* Certificate IV or Higher in Aboriginal and/or Torres Strait Primary Health or willing to complete* Willingness to obtain a prison clearance check**Desirable, but not mandatory:*** Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities**What Can we Offer?***At IUIH we recognise that we operate in a diverse community and welcome and encourage applications from all ages and genders, Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, the LGBTIQ+ community, veterans, refugees, and people with disability.** At IUIH 'Our Collectivity is Our Strength'.* We are a diverse and inclusive workplace that invites applications from all people.* Be part of Australia's largest community controlled Aboriginal Health Service.* Access to "Growing Our Own Opportunities" through in-house training and programs.* Weekly Staff Wellness activities and all staff 'Gather and Grow' yarns and award ceremonies.* Opportunities to attend community events such as NAIDOC, Rugby League Carnivals and more.* Onsite Presto's Café at our Windsor Office providing a range of hot and cold beverages and healthy breakfast and lunch options.### About UsIUIH is a Community Controlled Health Service (CCHS) that leads the planning, development and delivery of health, family wellbeing and social support services to the Aboriginal and Torres Strait Islander population of South East Queensland.
