Job Description

We're seeking a dynamic Stepped Care Coordinator to join our team in Central Queensland, Wide Bay, and the Sunshine Coast. In this role, you'll be responsible for guiding clients through a smooth and welcoming entry into our services, providing invaluable support on their journey to well-being.

About the Role

* You'll be the first point of contact for clients referred through the Head to Health Phone Line and other pathways.
* You'll oversee the care journey and collaborate with various teams to ensure each client receives the highest quality support.

Key Responsibilities

* Provide direct engagement and brief interventions to ensure smooth transitions and aftercare.
* Work closely with internal teams and external partners to deliver comprehensive care.
* Help clients navigate their care journey, ensuring a welcoming experience at every step.
* Oversee collaborative care planning, transitions between services, and aftercare.
* Build relationships with local hub partners and the lived experience workforce to foster community-driven care.

Ideal Candidate

* Strong leadership skills, taking charge of client care and team coordination.
* Extensive background in mental health and/or AOD settings, including knowledge of systems, legislation, and brief-intervention approaches.
* Experience working independently and as part of a care team in community-based recovery models.
* Proven ability to engage and collaborate with diverse services and professionals in the mental health space.

Qualifications

* Qualifications in psychology, social work, mental health nursing, counselling or a related field.
* A maintained relevant annual registration and/or membership of professional body (preferred).
