Job Summary
The Nurse Practitioner role is a key position within our healthcare team, offering quality and relevant care across the health continuum.
This includes health promotion, disease and injury prevention, curative, supportive, rehabilitative, and palliative care.
As a vital member of our interdisciplinary team, the NP will work in collaboration with physicians to ensure quality and relevant care for patients.
Responsibilities include:
* Integrating a broad knowledge base and critical appraisal in determining and communicating the diagnosis and patient needs.
* Applying advanced assessment techniques, critical thinking, and clinical decision-making skills when assessing patients.
* Systematically collecting and critically analyzing health data through holistic health assessments using multiple tools and sources of data.
* Completing prescriptions accurately and completely according to relevant legislation, standards, and policies.
* Ordering diagnostic imaging tests, laboratory, and other tests when clinically indicated and in accordance with the Nurse Practitioner Schedules and program guidelines for ordering.
* Promoting health and reducing the risk of complications, illness, and injury for patients.
* Ensuring that diagnostic tests are interpreted, and results are acted upon in an appropriate and timely manner.
* Monitoring the ongoing therapy of patients with chronic stable illnesses.
* Screening for the presence of chronic illnesses and working in collaboration with physicians who identified the presence of chronic illness in patients.
* Monitoring the effectiveness of interventions.
* Facilitating the patient's transition within and between healthcare settings.
* Ability to work independently and as part of the interdisciplinary team, making appropriate referrals.
* Documenting clinical data, assessment findings, diagnosis, plan of care, therapeutic intervention (including consent), patient response, and clinical rationale in a timely and accurate manner in the patient health record.
* Maintaining a positive working relationship with the interdisciplinary team.
Required Skills and Qualifications
* Bachelor's Degree in Nursing and successful completion of a postgraduate Nurse Practitioner program in Primary Health Care.
* Registered as a Nurse Practitioner with the Nurses Association of New Brunswick.
* Current Basic Life Saving certificate and maintain certification.
* At least 2 years' experience in primary health care setting in Community Health Centre or Collaborative Practice.
* Excellent interpersonal, oral, and written communication skills.
* Demonstrated organizational and leadership skills.
* Demonstrated ability to work collaboratively with other members of a team toward common goals.
* Demonstrated ability to function independently in the role with recognized decision-making and problem-solving skills.
* Demonstrated ability to critically analyze theory and research literature and apply to practice.
* Demonstrated ability to integrate a determinants of health approach into care delivery.
* Actively participates as a team member to provide comprehensive care to all patients.
* Must be able to work all shifts as assigned, maintaining a good attendance record.
* Adherence to professional ethical principles.
* Skillful use of computer tools and Microsoft Office Programs.
