 
        
        Job Title: Care Coordinator
We are seeking a dedicated and experienced Care Coordinator to join our team. The successful candidate will be responsible for ensuring that all residents receive the highest quality of clinical care.
Key Responsibilities:
 * Lead clinical and care service teams across the facility.
 * Assist in leading carer teams to meet all clinical needs of residents.
 * Manage all aspects of the assessment, documentation and submission of AN-ACC requirements.
 * Support effective documentation and user support of policies, procedures, and IT system requirements.
 * Coordinate assessment, care planning and review processes within the facility.
 * Support the admission of new residents and ensure optimal care assessments and initial consultations.
 * Record accurate information and ensure compliance with organisational and legislative requirements.
 * Manage and document clinical risk and commit to continuous improvements.
Requirements:
 * Minimum 1 year experience as a Care Coordinator or similar leadership position.
 * Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
 * Post graduate qualifications in aged care would be advantageous.
 * Experience and knowledge of Aged Care clinical assessments, care plans, AN-ACC and accreditation compliance.
 * Engaging style to influence and develop clinical leadership skills.
 * High level of computing skills to maintain documentation and reporting requirements.
 * A confident team player with excellent communication and interpersonal skills.
 * Ability to manage multiple stakeholders and have a balanced approach to emergency situations.
