ABOUT THE ROLE
The Carer Support Case Manager delivers the Support for Carers Program (SCP) across the Ovens Murray region, supporting carers to improve their health and wellbeing through flexible, person-centred supports.
In this role, you will engage carers, assess their needs and co-design tailored action plans, providing a mix of direct support, care coordination and brokerage. The role focuses on reducing barriers to access and supporting carers from diverse backgrounds, including young carers, culturally diverse communities, First Nations carers, LGBTIQASB+ carers, and those supporting people with mental health challenges.
Working collaboratively with internal teams and external providers, you will coordinate supports, respond to complex or urgent needs, and contribute to effective program delivery.
ABOUT YOU
You are an empathetic and capable practitioner with experience in case management, care coordination and needs assessment. You use a strengths-based, person-centred and trauma-informed approach, and can effectively engage carers from diverse backgrounds.
You build strong relationships, communicate clearly, and are confident managing competing priorities. You are comfortable coordinating a range of supports and navigating service systems.
You hold (or are working towards) relevant tertiary qualifications in Social Work, Community Services, Mental Health or a related field, with any required professional registration. Most importantly, you are passionate about creating inclusive, respectful and empowering environments where carers feel recognised, supported and valued.
