Nurse Practitioner Role
This is a role that involves providing medical care to residents in their homes. The position requires flexibility and the ability to work from home as well as visit residents in person.
* Clinical Leadership:
* Clinically assess, diagnose, and manage complex resident needs via telehealth or in-person visits.
* Lead clinical teams to deliver high-quality patient care.
* Collaborative Care:
* Work collaboratively with GPs, allied health professionals, and care-home staff to develop person-centred plans.
* Coordinate care to ensure seamless delivery of services.
* Resident Reviews & Telehealth:
* Conduct regular reviews of resident needs and update care plans accordingly.
* Provide telehealth services to residents who require it.
* Education & Mentoring:
* Develop and lead education programs for nursing staff and allied health professionals.
* Mentor nurses and allied health professionals to enhance their skills and knowledge.
* Governance & Research:
* Participate in clinical governance committees to ensure high-quality care.
* Contribute to research initiatives to improve patient outcomes.
