We are seeking a Level AHP2 Social Worker to provide high-quality assessment and treatment services for inpatient, home, and community rehabilitation patients across Northern Adelaide. You will use your clinical experience and professional expertise to plan, implement, and evaluate comprehensive, integrated services tailored to each client's needs.
Key Responsibilities
* Conduct assessments for rehabilitation patients across inpatient, home, and community settings
* Manage complex cases with multidisciplinary teams
* Provide support and guidance to patients and their families
Skills & Experience
* Degree in Social Work
* Eligible for AASW registration
* Proven experience in hospital or rehabilitation settings
* SA Health experience preferred but not essential
* Strong skills in assessments, discharge planning, and complex case management
* Fully vaccinated for COVID-19, including booster
* Competitive locum rates
* Full-time hours in a supportive healthcare environment
* Immediate start with potential contract extension
