The role of a Clinical Care Coordinator plays a pivotal part in ensuring the delivery of exceptional care to residents in our Residential Aged Care Homes.
This is an exciting opportunity for a dynamic and experienced leader to step into a leadership role, working alongside the Residential Care Manager to lead clinical operations and implement quality initiatives.
Key responsibilities include:
* Organising and facilitating case conferencing to partner with consumers and their representatives in the development of care provisions and plans;
* Providing expert case management alongside the Residential Care Manager;
* Monitoring and improving clinical outcomes through audits and quality initiatives;
* Supporting policy development and implementation for resident care;
* Leading, coaching and inspiring staff to deliver outstanding care;
To be successful in this role, you will need:
* A current AHPRA registration as a Registered Nurse;
* Proven clinical and leadership experience in residential aged care;
* Strong skills in assessments, care planning, and change management;
* A sound understanding of aged care funding models;
* A commitment to best practice and continuous improvement.
