As a key member of the Homeless Health Outreach Team, this Clinical Nurse Consultant will play a pivotal role in providing holistic care and support to individuals experiencing homelessness or unstable housing.
Main Responsibilities:
* Provide comprehensive care and support to individuals with complex needs
* Collaborate with multidisciplinary teams to develop and implement individualized care plans
* Conduct assessments and interventions to address mental health and addiction issues
Key Requirements:
* Registration as a Registered Nurse (RN) with the Australian Health Practitioner Regulation Agency (AHPRA)
* Postgraduate qualifications in Mental Health Nursing or a related field
* Experience working with marginalized populations and individuals with complex needs
