About the Program

Moving On provides culturally appropriate support and brokerage assistance to young people aged 16 – 25 years, who have transitioned to independence from OOHC in the Northern Territory.


Opportunity

As the Case Manager you will deliver strengths based, and culturally safe, information, assessment, referral, advocacy, support, group work and brokerage assistance to young people aged 16 – 25 years, who have left the care of Territory Families CEO. You will work collaboratively with various stakeholders to ensure comprehensive support and effective outcomes empowering them to achieve independence and reach their full potential.


Qualifications

* Hold relevant qualifications in Human Services/Social Sciences combined with direct experience working alongside high risk/young people & families in a community-based environment.
* Have a demonstrated understanding of the practical application of trauma informed, strengths-based practice and case management approaches, combined with strong youth engagement skills.
* Demonstrated ability to manage crisis situations and make good decisions under pressure.
* Ability to work respectfully and collaboratively with Aboriginal and Torres Strait Islander peoples, their families, communities, staff, and Aboriginal controlled organisations.

Please note: It is a requirement that prior to commencing in this role, you must provide 100 points of identification to confirm Australian citizenship or eligibility to work in Australia; possess a Working with Children (Ochre Card), a current National Criminal History Check (within 3 months) and a valid NT driver's licence.

Experience of working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.
