**About The Role**

We are seeking a motivated experienced nurse with excellent communication skills to join the Complex Care Response team. The Complex Care Response Nurse performs a key role in decreasing length of stay in acute settings by identifying clients who are eligible for Health Independence Program (HIP) services and assisting with streamlined referral from the acute setting to community care.This role is integral to bridging the gap between acute and Community services. Essential to the role is experience in general medical and surgical nursing as well as an understanding of community services. We operate from a person - centred care framework and align with the social model of health.

You are a dynamic Registered Nurse. With a genuine passion and interest in this unique opportunity and will commit to:

- A permanent role with a rotating roster
- Work across various Monash Health sites.
- Have sound skills in assessing and planning patient's community health discharge requirements.
- Current knowledge of various community health support services.

- Ensuring you work within the scope of practice as a Registered Nurse, and escalating to the appropriate management line, Nurse Practitioners and Medical Professionals when required.
- Completing all mandatory and competency assessment training.
- On a personal level, you are enthusiastic and motivated and demonstrate excellent communication and interpersonal skills. You are willing to commit to team work and ongoing professional development that align with the Monash Health's values and vision. **What You Need**:
Essential:

- Demonstrated recent experience as a Registered Nurse with at least 5 years post graduate experience.
- Current AHPRA registration
- Australian working rights
- Valid Driver's licence
- Current Working with Children's Check
- Current Police Check
- Desirable_:

- Previous Community Nursing experience
- Post-graduate qualifications
