We offer specialised services in alcohol and other drug treatment, mental health, and wellbeing, through counselling, group support, residential rehabilitation, and education—reaching communities across the Perth metro, Peel, South West, and Great Southern regions.

About the Role

As an Alcohol and Other Drugs (AOD) Case Worker, you will support individuals as they transition from hospital back into the community. This role focuses on delivering ongoing, person-centred support to promote recovery, improve wellbeing, and strengthen connections with community-based services following discharge. As part of a wraparound care model, you will help bridge the gap between clinical mental health services and community organisations, supporting improved mental health outcomes and the effective management of alcohol and other drug use. This role is classified at Level 4 under the Social, Community, Home Care and Disability Services Industry Award 2010.

What We're Looking For

You're more than your qualifications—you're someone who brings heart, empathy, professionalism and a strong commitment to supporting clients in improving their wellbeing. The ideal candidate will meet the following selection criteria:

* Tertiary qualifications (or equivalent) in Social Work, Behavioural Sciences, Health Sciences, or a related discipline.
* Demonstrated experience supporting people experiencing alcohol and other drug (AOD) and/or mental health challenges.
* Current valid driver's licence and access to a reliable vehicle, with the ability to travel to various locations as required.
* A strong commitment to culturally safe practice, with the ability to engage respectfully and effectively with people from diverse cultural backgrounds.
* Proven experience in case documentation, reporting, and maintaining accurate client records.
* Experience working with individuals presenting with complex and co-occurring needs, including mental health and substance use concerns.
