Central Gippsland Health (CGH) is a sub-regional health service and the major provider of health and aged care services in the Wellington Shire. CGH is located in the City of Sale which is 215 km east of Melbourne. The City of Sale is an ideal access point for the magnificent Gippsland Lakes, 90 Mile Beach and the Victorian Alps.
CGH provides acute care as part of an integrated service delivery model across Wellington Shire and beyond. The service provides beds across three campuses (Sale, Maffra and Heyfield). Acute services include a full-time emergency department, critical care unit, neonatal critical care unit, three operating theatres, day procedure unit, oncology and dialysis services in addition to general medical and services and sub-acute services including rehabilitation.
CGH is also a leading teaching hospital, affiliated with Monash University's School of Rural Health and provides placements and clinical experience for students.
About Department
Wilson Lodge is a residential aged care facility with 50 beds. Wilson Lodge caters for emergency respite, short term planned respite as well as long term permanent placement to meet the individual needs of the community to which it serves. All residents are welcome regardless of their care needs. Wilson Lodge staff work with individuals to provide high quality care in line with aged care quality and safety commission guidelines and the national aged care standards.
About the Role
The purpose is to provide social, recreational, diversional and intellectual activities and programs which reflect the assessed needs of residents within the service in conjunction with both internal and external stakeholders.
This will occur within the principles of care coordination, where patients/clients/residents have access to a highly coordinated health care and community support system.
The Diversional Therapist will work collaboratively as part of a multidisciplinary team which includes other members of the care team and all staff of the environment in which they are working. This will occur in the aged care setting.
Duties
* Provide support and facilitation of learning opportunities to undergraduate students from all disciplines.
* Facilitate and assist with therapeutic groups and/or educational programs.
* Participate in information gathering to assist in assessment process to develop individual care plans.
* Evaluate the ongoing effectiveness of therapy care plans and provide appropriate feedback to the Registered Nurses.
* Work with clients, their families and carers to carry out person-centred programs.
* Work collaboratively with all members of the multidisciplinary team to facilitate integrated person-centred care leading to enhancement of resident's quality of life.
* Liaise, network and collaborate with other service providers. Advocate for residents and their families including assisting residents to be independent.
* Comply with guidelines and relevant accountability requirements including statistical data collection requirements.
* Work with volunteers and provide support and guidance.
Qualifications, Skills and Experience
* Certificate 3 / Diploma or willingness to obtain.
* Strong communication, organisational skills and ability to work independently.
