About the role
The Street to Home program provides assertive outreach to individuals experiencing rough sleeping and chronic homelessness, using a Housing First model that prioritises rapid access to safe, long‐term housing. This model is supported by intensive case management and strong partnerships across local services, enabling a coordinated, multi‐agency response that promotes sustained housing stability and community reintegration.
In this role, you will deliver mobile, outreach and centre‐based case management support to individuals and families experiencing vulnerability in the Townsville region. You will conduct holistic assessments, develop case plans and collaborate with a wide range of service providers to ensure clients receive the support they need. You will also contribute to co‐design processes and seek opportunities to innovate and strengthen Red Cross's response to client needs.
What you will bring
* Sound casework support and case management experience including holistic assessment of client needs and development of case plans
* Experience in working competently with staff and clients from cultural and linguistically diverse backgrounds with respect and humility
* Well‐developed interpersonal and written communication skills
* Proficiency in using Microsoft office
A proven ability to contribute to interagency collaboration, and confidence leading joint efforts with internal and external stakeholders—including statutory and law enforcement agencies.

This role is covered by the Social, Community, Home Care and Disability Services Award - Level 4. Salary from $88,090.08+ superannuation + salary packaging.
