About Sunraysia Community Health Services

Sunraysia Community Health Service (SCHS) is a dynamic organisation promoting health, wellbeing and independence in a social model of health. The organisation is a source of primary and preventive care for our community members who are at risk of poor health outcomes, and experience acute health disparities. SCHS understands the influence of the Social Determinants of Health upon health inequities experienced by community members and work with consumers and community members, to structure their care around the context in which they live and work.

The Victorian Palliative Care Network consists of 8 Palliative Care consortia who work with and support palliative care services in their health regions. The network is critical to the Victorian health system and plays a key role in implementing palliative care policy and effective palliative care service delivery, including quality end-of-life care in community and inpatient settings. The Loddon Mallee Palliative Care Consortia supports the large and diverse Loddon Mallee region, with SCHS responsible for delivering the Northern region consultancy service, covering Mildura, Robinvale and Ouyen.

SCHS Community Palliative Care services delivers compassionate, coordinated care for individuals with advanced chronic and life-limiting illnesses. The service provides community-based management of palliative care clients including physical and psychological, social and spiritual care.

POSITION PURPOSE:

The Loddon Mallee Regional Palliative Care Consultancy Service provides 'in reach' and 'outreach' consultancy advice and support to hospital-based staff, generalist community-based healthcare professionals and community palliative care services.

The Clinical Nurse Consultant will provide specialist nursing advice and clinical leadership in assessment and symptom management, clinical decision-making, advance care planning, patient and family communication, and end of life care.

KEY SELECTION CRITERIA:

Mandatory:

* Current registration with AHPRA as a Registered Nurse or Nurse Practitioner.
* Minimum of 4 years' senior experience working in a Palliative Care environment
* Postgraduate qualification in Palliative care, or willingness to work toward, including a pathway to Nurse Practitioner candidacy.
* Demonstrated leadership experience within a multi-disciplinary setting.
* Demonstrated experience in the provision of education about palliative care for health professionals.
* Holds a current Victorian Driver's License.

Preferred:

* Proven ability to implement changes and develop direction in clinical practice using evidence-based research.
* Demonstrated negotiation, collaboration, problem solving and analytical skills.
