About the role

The new Palliative Care Nurse Practitioner role will provide advanced, person-centred care across both acute and community settings using a hub-and-spoke model that adapts to the evolving needs of the Barrington Coast community. This hybrid model combines face-to-face consultations with virtual care to enhance accessibility and continuity of service. Working collaboratively with a multidisciplinary team including Palliative Care Medical Officers, Clinical Nurse Consultants (CNC), Clinical Nurse Specialists (CNS2 and CNS1), and Registered Nurses the Nurse Practitioner will lead evidence-based practice to deliver seamless, high-quality palliative care. This role strengthens integration between hospital and community services, improves equity of access, and supports best-practice outcomes aligned with contemporary palliative care standards.

This is a newly established model of care, offering a unique opportunity for you to play a pivotal role in shaping and refining this innovative approach.

The successful candidate will demonstrate advanced clinical expertise in palliative care, exceptional communication and collaboration skills, and the ability to work autonomously while fostering team integration across acute and community settings. A high level of adaptability and problem-solving is essential to support the evolving hybrid model of care. Candidates should have significant experience in palliative care, preferably at an advanced practice level, and hold Nurse Practitioner endorsement or be eligible for endorsement. Additional skills in virtual care delivery, clinical leadership, and evidence-based practice implementation will be highly regarded.

Requirements

* Current Registration as a Registered Nurse with AHPRA and current endorsement as a Nurse Practitioner or, if transitional, evidence of substantial progress towards endorsement. If not endorsed, the Transitional Nurse Practitioner will be appointed temporarily (up to 3 years) to work towards endorsement as a Nurse Practitioner.
* Eligibility to Drive in NSW and ability to travel as required.
