About the Role
This is a place-based position where you will work directly with individuals to empower them to achieve their goals and live their best lives.
* Support individuals throughout their NDIS journey, assisting with scheme access, plan development, implementation of funded supports and plan reviews.
* Help build confidence, independence and informal support networks for participants.
* Ensure timely and high-quality service delivery in line with performance targets.
* Establish partnerships with local organizations, attend events and promote inclusion through grassroots engagement.
* Utilize NDIS and organizational systems to document participant interactions, monitor progress and identify service gaps.
* Work within the framework of NDIA guidelines, the PitC Deed and internal standards.
* Participate in team meetings, regional planning and initiatives promoting inclusion, cultural safety and continuous improvement.

Required Skills & Experience
To succeed in this role, you will require:
* Experience supporting people with disabilities in community services, social work or similar settings.
* Strong understanding of the NDIS participant pathway and planning processes.
* Proven ability to manage caseloads and meet service delivery targets.
* Excellent communication, interpersonal and problem-solving skills.
* Strong community engagement and stakeholder relationship skills.
* Experience in data entry, reporting and case management systems.
* Ability to work independently in community-based settings.
* Commitment to person-centered practice, inclusion and continuous improvement.
