Remuneration: $32.78 - $47.39 per hour
Hours Per Week: 38
Location: Mount Druitt Hospital

What you'll be doing
In this role, you will provide culturally safe, holistic, and client-centred support to Aboriginal individuals and families impacted by substance use. You will play a key role in strengthening engagement, improving access to care, and supporting recovery outcomes.

* Deliver culturally appropriate health education, advocacy, and support
* Coordinate care and referrals across health, housing, and community services
* Collaborate with multidisciplinary teams on care planning and case reviews
* Support discharge planning, follow-up, and continuity of care
* Engage with community and promote culturally safe practices within the service

Essential Requirements:

* License: NSW Drivers License
* Qualification: Holds, or is working towards, a minimum Certificate III qualification in Aboriginal Primary Health Care, or a minimum Certificate III health qualification relevant to the area of care in which the Aboriginal Health Worker is employed
* Evidence of Diversity claim if applicable (state which category): To be a person of Aboriginal descent. Aboriginal is a genuine occupational qualification and is authorized under Section 14 (d) of the NSW Anti-Discrimination Act 1977).

Selection Criteria:

* Demonstrated understanding of the Social Determinations of Aboriginal Health particularly in relation to substance use and recovery
* Proven experience providing culturally safe, client-centred care to Aboriginal people within health, community, or social service settings
* Demonstrated ability to support client assessment, care planning, advocacy, and referral processes within a multidisciplinary team environment
* Proven experience in understanding and respecting cultural protocols related to Men and Women's Business within a health setting
* Strong interpersonal and communication skills, with the ability to build trust and collaborate effectively across services and communities
* Sound knowledge of local Aboriginal community networks, services. And supports across WSLHD
* Current NSW Driver's Licence and willingness to travel across WSLHD facilities.
