A Support Worker plays a vital role in empowering individuals with disabilities to live as independently and thoroughly as possible. Acting as a daily point of support, the Support Worker assists clients with personal care, household tasks, and community participation, always using a person-centred approach. This role involves building strong, respectful relationships with clients and their families, promoting choice and dignity, and supporting clients to achieve their goals. Support Workers collaborate with internal teams and external professionals to ensure coordinated care, maintain accurate records, and uphold all relevant policies and compliance standards. Their work is crucial in creating a safe, inclusive, and supportive environment that enables clients to thrive and actively participate in their communities.

RESPONSIBILITIES AND DUTIES

Act as the first point of contact for new participants, families, and representatives, providing information about available disability services and guiding them through the intake process.

Conduct initial assessments and screenings to determine participant needs and eligibility for services.

Accurately collect, enter, and manage participant information and documentation in organisational systems.

Coordinate with internal departments to facilitate seamless service commencement and participant onboarding.

Support participants and their families in understanding service options and benefits, ensuring a person-centred approach.

Maintain efficient communication with all stakeholders, including participants, families, external providers, and government agencies.

Prepare reports and maintain up-to-date records as required by management.

Represent the organisation at community events, expos, and forums to promote services and build brand awareness.

Support marketing and business development initiatives, including generating and converting leads through sales and promotional activities.

Handle participant feedback and complaints, ensuring prompt and professional resolution.

Ensure a smooth and positive intake experience for all new participants, from initial enquiry to service commencement.

Uphold organisational values and promote the dignity, rights, and independence of people with disabilities.

Adhere to intake systems, policies, and NDIS guidelines to ensure compliance and quality service delivery.

Build and maintain strong relationships with participants, support coordinators, plan managers, and other stakeholders.

Act as an ambassador for the organisation, professionally representing its services in the community and at relevant events.

Identify opportunities for service improvement and contribute to continuous quality enhancement.

ABOUT YOU


Essential Qualifications

Education:

Certificate III or IV in Disability, Community Services, Social Work, Human Services, or a related field

Experience:

Previous experience in an intake, customer service, case management, or administrative role within the disability, community, or health sector

Experience working with people with disabilities and/or their families is highly regarded

Knowledge & Skills:

Understanding of the National Disability Insurance Scheme (NDIS) and relevant legislation

Strong knowledge of person-centred practice and principles

Excellent verbal and written communication skills

High-level interpersonal and relationship-building skills

Strong organisational and time management abilities

Proficiency in Microsoft Office and client management systems (e.g., CRM software)

Personal Attributes:

Empathetic, patient, and culturally sensitive

Strong attention to detail and accuracy

Ability to work independently and as part of a team

Commitment to confidentiality and professional ethics


Desirable (Preferred) Qualifications

Diploma or higher qualification in Disability, Social Work, Community Services, or related discipline

Experience with NDIS processes, intake systems, or similar client management frameworks

First Aid certificate and/or Mental Health First Aid

Multilingual skills or experience working with diverse communities


Mandatory Requirements

Current National Police Check (or willingness to obtain)

Working With Children Check (WWCC)

Valid driver's licence

Evidence of COVID-19 vaccination
