Cancer Care Coordinator - Wagga Wagga
Job Description:
The Cancer Care Coordinator will work as part of the Oncology team in Wagga Wagga, Griffith or Young. This role involves collaboration with various services including LHD Health Services, the Riverina Cancer Care Centre, tertiary referral cancer centres, Community Health, GPs, Medical Specialists and other cancer service providers to increase access and ensure effective coordination of care.
About the Role:
This position will focus on improving the quality of treatment and life for cancer patients by streamlining the treatment pathway. The Cancer Care Coordinator will have an opportunity to enhance patient advocacy, provide clinical leadership and practice autonomously within a multidisciplinary team environment.
Funding and Partnerships:
This position is funded by the Cancer Institute NSW to establish clear referral pathways with local GPs and service providers for cancer patients. This initiative aims to assist patients accessing cancer treatment and improve continuity of care within and external to the Local Health District.

Required Skills and Qualifications:
Clinical Nurse Specialist Grade 2 (CNS2) or Clinical Midwife Specialist Grade 2 (CMS2)
