Pramana Medical is seeking an experienced Social Worker to join their award-winning team.
Pramana Medical Centre (PMC) is a family operated GP clinic located in Gosnells, WA. PMC is dedicated to quality improvement and improving health outcomes for their patients. In this role you will form an integral part of a unique practice, with a well-established and integrated multidisciplinary team providing opportunities to enhance your skills and learn from a very diverse range of allied health, doctors, nurses, Nurse Practitioners and specialists.
PMC has a very well-established patient following, with the majority of patients being First Nations people or people from marginalized backgrounds. We desirably would like a social worker with at least three years experience.
About the role:
In this role you would be expected to assist with client advocacy, counseling and support, close collaboration with our various allied health, GP's and nurse practitioners, crisis intervention which includes emergency housing support and DCP liaison, maintaining sufficient documentation and communication with patients. Assisting with various forms such as NDIS, DSP and support letters for patients in need. Willingness to be flexible and adaptable to clinic needs and to maintain cultural awareness training.
As part of this role, you will work closely with our GPs, Nurse Practitioners and Specialists to coordinate patient care.
This position is part time working weekdays, 1-3 days a week. Our opening hours are 9am-5:30pm weekdays. There is no afterhours work required.
About you:
The right candidate for this role will be:
* Qualified and Experienced Social Worker - At least 3 years experience as this is a fast paced role that requires critical thinking and knowledge of community resources
* Dedicated to quality improvement and likes to try new things
* Able to work autonomously and as a part of a team
* Dynamic and able to work in different work environments
* Has good time management and works well under pressure
* Holds a valid driver's license
* AASW membership is preferred but not essential
* Must hold a valid Working with Children Check + Police Clearance
