The Dementia Services Team Lead provides service leadership, team coordination, and governance oversight within the Dementia Together program, ensuring high quality, person-centred dementia support aligned with CatholicCare's mission, values, and organisational frameworks.
The role focuses on leading people, strengthening workforce capability, ensuring quality and compliance, and supporting consistent service delivery across the multidisciplinary team. While an understanding of dementia-specific clinical principles is important, the role's emphasis is on leadership, supervision, coaching, and service governance, rather than direct clinical practices.
The primary objectives of the Dementia Services Team Lead role includes:
* Provide strong day-to-day operational leadership, direction and support to Registered Nurses, Case Managers, Support Workers and other team members;
* Build workforce capability through supervision, coaching, induction and mentoring;
* Ensure service delivery quality through oversight of documentation, compliance, risk management and adherence to governance frameworks;
* Coordinate workforce planning, resource allocation and workload management to meet client and program requirements;
* Support client outcomes through high-quality service coordination, ensuring team members deliver services consistent with best practice dementia support;
* Lead quality improvement initiatives, including review of incidents, audits and practice development;
* Strengthen stakeholder relationships to improve integration, communication and continuity of care; and
* Contribute to organisational strategy and continuous improvement initiatives.
Qualifications and Experience
* Bachelor degree in Nursing, Health, Community Services or related discipline;
* Minimum 5 years' experience in community services, aged care or health settings;
* Current C or CA class Driver's license (QLD);
* Experience supervising or mentoring multidisciplinary teams;
* Understanding of clinical governance principles (formal clinical registration desirable but not required);
* Experience in dementia support, aged care programs or community-based service delivery.

Other Requirements
The role may occasionally require work at different locations or outside usual hours to support client or service delivery needs.
The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
