Job Title: Case Manager - Community Support Specialist
About the Role
We are seeking a skilled and compassionate Case Manager - Community Support Specialist to join our team. As a key member of our STARH (Supporting Those At Risk of Homelessness) team, you will be working closely with individuals and families at risk of homelessness or who are experiencing homelessness.
Your Key Responsibilities Will Include:
* To provide high-quality person-centred case management services to clients, assessing their needs and developing tailored plans to support them in achieving independence, safety, and meaningful outcomes.
* To work collaboratively with other service providers, community partners, and relevant stakeholders to ensure seamless delivery of support services.
* To maintain accurate records of client interactions and progress through regular updates on case management notes.
You'll Be Well Suited For This Role If You Have:
* A degree qualification in human services or a related field required with the required level of experience. We're looking for candidates who have expertise in trauma-informed practice principles!
* Experience working with diverse populations including people from CALD backgrounds & those living w/ mental health conditions
