On-site facilities include a gym, art centre, chapel, dance room, theatre, library, several hospitality outlets, village shed, barber shop, hairdresser, medical consultation room, grocer, purpose designed meeting spaces and a dog park.About the RoleReporting to the Clinical Case Manager Lead, the Clinical Case Manager supports integrated and coordinated resident care and service requirements, including pre-entry clinical assessment, move-in support, care planning, care delivery, care evaluation, referral and accessing externally funded care (with associated documentation), coordination of support for residents and networking.Role Responsibilities:The Clinical Case Manager has an important leadership role within the care team and liaises with a wide range of internal and external stakeholders. The position works Monday to Friday 0830am - 5pm with competitive rates in the industry plus superannuation. Duties include but are not limited to:Streamlining care for your portfolio of residentsConducting timely and comprehensive clinical assessmentsOverseeing resident support to determine precise care needsFacilitating smooth transitions to LDK Homecare AP where neededWelcoming new residents to the Village, introducing them to the care team and assistive technologiesAssisting residents in accessing externally funded careCoordinating aged care assessments through ACAT/MyAgedCareNavigating Commonwealth Home Care Packages and Support ProgramsHandling Veterans' care coordination, including GP communication and documentationAssessing eligibility for external care servicesCollaborating with the Clinical Case Manager Lead to meet resident goals and preferencesOur Employee Value PropositionLDK villages have established a reputation of being ‘truly happy places’ for residents.
