About the Role
The Clinical Care Coordinator will ensure the home remains in compliance with the requirements of the aged care standards.
Working closely with the Home Manager, you will ensure that the Home embodies a strong diversity culture that advances the frontiers of aged living through innovation and integration of clinical practice with lifestyle pursuits, hospitality and design.
Key Responsibilities
Ensure clinical and specialised nursing care needs of residents are met and documented in individual resident care plans by clinical staff.
Maintain residents and their loved ones at the centre of decision making, ensuring their needs, choices, independence, dignity and identified risks are considered.
Oversee and implement all scheduled mandatory and voluntary ACFI reappraisals as required.
Hold RN's and EN's accountable for daily case reporting, care planning and addressing patient concerns on a day-to-day basis.
Ensure regular clinical sweeps are performed and all gaps are addressed in a timely manner.
About You
Current registration as a Registered Nurse with AHPRA – no restrictions.
Experience in a senior management role within the aged care sector with a successful and proven track record.
Above-average knowledge and understanding of the aged care sector with the ability to identify changing trends and apply best practice.
Understanding of and ability to manage human resource issues with effective negotiation skills.
Ability to use information technology and programs such as Word, Excel, etc., and to learn new skills as required.
