OverviewClient Relations Manager - Home Care Packages, AQ.
This full-time role is based onsite at the Arundel Multi Service Centre with occasional travel to the head office in Upper Mount Gravatt.
Closing Date: 31 Oct 2025 AEDT.Alzheimer's Queensland (AQ) is a leader in aged care and dementia services, delivering personalised care to empower older Australians to remain independent and engaged within their communities.
What You'll Be DoingBuild strong relationships with clients and support onboarding, ensuring care plans are delivered with compassion and excellence.Identify and cultivate business growth opportunities, foster partnerships, and drive revenue in collaboration with stakeholders.Assist with the transition from the Home Care Program to the current Support at Home Program from 1 November 2025.Provide HCP case coordination, including in-home assessments, referrals, care plan reviews, and budgeting.Engage new clients and facilitate smooth transitions into the Support at Home program.Maintain solid relationships with existing clients to ensure high satisfaction and quality outcomes.Coordinate timely assessments, care plans, and goal-setting tailored to individual needs.Oversee client budgets to ensure funds are used appropriately and compliant with Department guidelines.Support and lead a care team to ensure services are delivered effectively and respectfully.Collaborate with internal teams to ensure operational and regulatory compliance.Respond to client inquiries, manage changes in care needs, and ensure prompt service delivery.
About YouExtensive knowledge of the home care standards and understanding of the Commonwealth Home Support and Consumer Directed Care programs.Strong client relationship management and interpersonal skills.Proven experience in consumer directed care and budget management.Diploma or higher qualification in aged care, community services, health, or a related field.Client-focused mindset with excellent verbal and written communication skills.Current driver's licence and access to an insured motor vehicle.National Police Check (or ability to obtain).
