Employment with Mid North Coast LHD requires candidates to hold a valid authority to work in Australia (Aust Work Rights Visa).
What you'll be doing
To provide an effective and efficient Social Work service that addresses the psychosocial aspects of care for patient, as identified.
To provide intervention including support, education and resources to people in the context of their bio-psycho-social situation, to assist in dealing with health events.
About You
You are a qualified Social Worker, holding a degree in Social Work or an equivalent qualification that meets the eligibility requirements for AASW membership. You bring a strong understanding of the Social Work role within the healthcare environment, demonstrating insight into multidisciplinary practice and the complexities of patient‑centred care.
You have proven capability in conducting comprehensive psychosocial assessments, formulating intervention plans, and delivering evidence‑informed support to individuals and families. Your practice reflects a commitment to ethical standards, cultural sensitivity, and collaborative care.
You work effectively within interdisciplinary teams, contributing to shared goals while respecting the expertise of other disciplines. You are skilled in organising and facilitating family meetings, navigating sensitive conversations, and coordinating complex case management plans that support continuity of care.
Your communication skills—verbal, written, and digital—are well‑developed, enabling you to document accurately, engage meaningfully with clients and colleagues, and manage a high-volume workload with professionalism and reliability. You approach your work with initiative, accountability, and a strong commitment to improving outcomes for the people you support.

Salary will be accordance with NSW Health State Awards for the advertised classification/s. These awards are available at http://www.health.nsw.gov.au/careers/conditions/pages/default.aspx.
