Hourly rate: $44.78 - 48.99 per hour plus Not‐for‐Profit Salary Packaging to maximise your take‐home pay.
About the role
As a Care Partner you will provide older adults, their families and carers with responsive care and case management, ensuring tailored services and support to meet their needs.
On a typical day, you will liaise with older persons by phone or in their home to follow up on care requirements, and engage in conversations about achieving happy, fulfilled, and independent lives.
Responsibilities
* Utilise case management skills to undertake initial and ongoing comprehensive assessments and care planning for clients at home.
* Manage the delivery of services (psychosocial, medical, physical, and vulnerabilities) within the customer's individualised budget.
* Build and maintain relationships with colleagues, service networks, and community agencies to support the program and customers.
* Follow up with older adults and families to support their goals and maintain independence in their homes.
Qualifications
* Australian Driver's Licence.
* At least 2 years' experience in case management in home and community‐based settings.
* Qualifications in Aged Care, Health, Community Services or related discipline (highly desirable).
* Experience building professional relationships with customers, supporters and external service networks.
* Understanding of My Aged Care and the Aged Care Quality Standards.
