At Alfred Health, we have a unique opportunity for a Social Worker to join our dynamic team. In this exciting role, you will provide high-quality assessment and management of patients within our Acquired Brain Injury (ABI) Rehabilitation Centre.
The ABI Rehabilitation Centre is a state-wide specialist service that provides rehabilitation services for people with moderate to severe acquired brain injuries. As a Grade 2 clinician, you will be responsible for providing individual and family counselling, including grief counselling, adjustment to illness, risk management, and coordination of interdisciplinary family meetings.
Working in a community rehabilitation setting, you will contribute towards patients' rehabilitation following a brain injury, including goal setting and goal attainment. You will also support patients within their homes, often following their discharge from hospital with a newly acquired brain injury.
As part of an interdisciplinary team approach, you will work collaboratively with other healthcare professionals to achieve optimal patient care and outcomes and to minimize clinical risk. Additionally, you will contribute to quality improvement and research activities within the ABI service and promote a culture of ongoing learning and development within the team.

Qualifications and Experience
To be successful in this role, you will require:
* A recognised degree in Social work
* Eligibility for membership of AASW
* Previous experience in a community rehabilitation setting is highly desirable
