Your responsibilities include:
Assessing, planning, and coordinating clinical care for older Australians and their families Collaborating with multidisciplinary teams to implement best-practice care Ensuring care plans are current, evidence-based, and tailored to each individual Promoting safety, dignity, and well-being while supporting clients to remain in their homes Maintaining accurate documentation and meeting compliance standards This role provides a unique opportunity to build genuine relationships with clients while delivering meaningful care.
About You You are a compassionate, client-focused professional who:
Is registered with AHPRA as a Registered Nurse with a current Practising Certificate Has skills in clinical assessment, care planning, and case coordination Holds a current CPR certificate and a valid driver's license Is comfortable using mobile devices for clinical documentation Has experience in aged care or community settings (preferred) Possesses a current National Police Certificate and NDIS Worker Screening Check (or ability to obtain them) Can undergo a National Personal Insolvency Check About Us We are one of Australia's largest not-for-profit organisations with over 140 years of care and compassion.
