Gateway Health is a community health service providing health and welfare services to individuals, families and communities across Northeast Victoria and parts of southern New south Wales. We are a not-for-profit Company limited by guarantee and a registered Community Health Service under the Health Services Act 1988 (Vic).
The organisation employs over 380 staff providing a range of primary health and community services across the Ovens Murray region in Northeast Victoria and parts of Southern New South Wales. We do this by providing health care and support to individuals and communities in times of need, and by actively working with our partners to address the social and environmental determinants of health.
ABOUT THE ROLE
The Care Finders program is a national program that provides specialist and intensive support and assistance to people who are eligible for aged care services but are not engaged with any services or have family, friends or broader networks to help. Gateway Health (GH) has been contracted to provide the Care Finders program in the Local Government Areas of Albury, Alpine, Benalla, Indigo, Towong, Wangaratta and Wodonga. Care Finders use an assertive outreach model to identify and offer intensive support and assistance to eligible clients in these regions.
The Care Finder Coordinator is responsible for providing leadership to the Care Finder Team. They report directly to the General Manager Community Care. They will ensure the Care Finder team provides best practice and meets quality standards and contractual obligations. As a senior practitioner the Care Finder Coordinator is expected to provide expert advice, including secondary consultation to Gateway Health staff members when expertise is required.
ABOUT YOU
You are a dedicated professional with relevant tertiary education (Social Welfare, Community Services, Aged Care, Disability, Mental health, Youth Work or other relevant discipline) at a minimum Certificate IV level.
You have a strong understanding of the aged care system and the challenges faced by older people and their families/carers, including those from diverse backgrounds. Your practice is grounded in person-centred care, and you use a variety of communication strategies to ensure clients are fully engaged and empowered to make informed choices.
You have demonstrated leadership and supervision capabilities, with the ability to support and guide a team effectively.
Above all, you are passionate about improving access to aged care and community services and have the knowledge, empathy, and coordination skills to make a meaningful impact.
