About the role
Working as part of a multidisciplinary team, this position will be responsible for coordinating the care of clients seeking services within their specialty area and providing expert and quality clinical services in collaboration with consumers, carers and other care providers, using an integrated approach.
This position will undertake the role of Assessor for the Single Aged Care Assessment Service and to provide client-end assessment and provide a counselling role by supporting people living with dementia who have recently been diagnosed to manage their condition and maximise independence.
The Aged Care Division delivers a range of clinical and non-clinical services across the entire Local Health District to support Older people in their families to enable them remain supported in their home.
