Housing Case Manager Job Overview
This role focuses on delivering culturally safe, trauma-informed case management to support clients in their transition to stable housing.
Key Responsibilities
* Deliver case management services using a person-centred approach to support clients in achieving stable housing.
* Conduct intake assessments and develop tailored plans for approximately 20–25 clients while providing additional support as needed.
* Assist with tenancy applications and provide practical help in setting up and maintaining housing through liaison with housing providers and government agencies.
* Refer clients to relevant services (e.g., AOD, mental health, financial counselling, childcare) and advocate for ongoing support and wellbeing.
* Respond professionally to crises, identify risks, and implement strategies to manage safety and engagement.
* Maintain accurate records and contribute to internal reporting, continuous improvement processes, and team reviews.
* Engage in culturally sensitive practice, developing strong partnerships with Aboriginal communities and services across the region.
