Job Title: Clinical Care Coordinator

In partnership with Wellways and Clarity Health Care, we will soon be opening the Western NSW Medicare Mental Health Centre, with locations in Dubbo and Bathurst. The Medicare Mental Health Centres, formerly Head to Health, are an important part of Australia's reformed mental health and wellbeing system.
The Medicare Mental Health Centre is supported by funding from Western NSW Primary Health Network through the Australian Government's PHN Program. The centre will provide a free, safe and welcoming space to access mental health advice, supports and services. Walk-ins will be welcomed, services are confidential, and no appointment or referral will be required.
The services in Dubbo and Bathurst are funded by the Australian Government, to provide a streamlined approach to support better mental health and social wellbeing across the country. The service is tailored to the needs of the community in regional NSW and provides a combination of peer-led recovery and clinical support.
The Medicare Mental Health Centre provides a safe, welcoming and supportive environment for people experiencing psychological distress, providing an alternative to presenting to the Emergency Department for low risk individuals, and allowing them to access support from trained mental health professionals within their community, outside a clinical setting.
About the Role:
Deliver high quality evidence based clinical care as a member of the multidisciplinary team to achieve better health outcomes for adults aged 18 years and over who experience mental health concerns and their families and carers.
* Conduct intake and assessments. Including risk assessments, care treatment planning, care coordination with internal & external services, and provide brief psychological intervention.
Required Skills and Qualifications:
To be successful in this role you must have one of the following qualifications:
* Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist OR AASW registration/eligibility as a Social Worker.
* Must have flexibility to work afternoons & evenings, weekends and public holidays.
* Knowledge & understanding of the theory underpinning evidence-based interventions & clinical practice as well as demonstrated excellence in the practical application of such, in the areas of mental health and alcohol and drug use.
* Highly developed communication (including written) and interpersonal skills.
* The ability to engage with people, advocate on their behalf and establish and maintain effective relationships with staff, clients, families & friends and stakeholders.
* Experience in the use of electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications.
* Must be eligible to apply/currently hold a WWCC.
* Must hold/be willing to apply for NDIS worker screening clearance.
