Meaningful Engagement Coordinator
An exciting opportunity has arisen to play a key role in our care community's new Meaningful Life Model of Care.
The coordinator will be responsible for the development, implementation and coordination of meaningful engagement that gives daily purpose to our residents. This will be delivered through various activities and purposeful engagement that supports their holistic wellbeing.
* Develop and implement purposeful engagement across the care community, meeting individual residents' goals, needs and preferences while promoting choice.
* Provide leadership, mentorship and support to the meaningful life team, volunteers and students in the care community.
* Build connections with the local community to deliver high-quality, innovative and person-centred engagement.
* Coordinate and manage all meaningful life assessments, care plans, residents' profiles and documentation.
* Attend and provide reports/feedback at resident, relative and care community team meetings.
* Coordinate regular resident and relative surveys/audits, reporting progress and outcomes.
* Coordinate rostering, training and professional development for direct reports.
* Respond promptly to any complaints or concerns raised.
Requirements:
- Hold a Certificate III in Aged Care (or equivalent) or Certificate IV in Leisure and Health (or equivalent).
- A qualification in dementia care is desirable.
- Demonstrate an understanding of Aged Care Quality Standards.
- Excellent communication and organisational skills, with strong computer and customer service skills.
