Job Overview

We are seeking a highly skilled and experienced Disability Services Manager to join our team. As a key member of our management team, you will be responsible for providing exceptional leadership and management in the delivery of disability services.

Your primary focus will be on client services management, ensuring high-quality customer experiences and effective care plan review and management.

You will work closely with clients and their families to identify assessment care needs and support services required to promote enablement, dignity, independence, and choice.

This role requires a strong understanding of community services coordination and case management principles, as well as experience in managing SIL/disability accommodations.

Key Responsibilities

* Care Plan Management:
* Fundamental to this role is the development and implementation of comprehensive care plans that meet the unique needs of each client.
* Client Relationship Management:
* You will engage in regular communication with clients and their families to ensure they receive the highest level of support and care.
* Critical Functions:
* The Disability Services Manager will oversee critical functions such as point of contact for clients, management and improvement of client experience through engagement of care teams and stakeholders, and oversight of funding utilisation.
* Service Level Monitoring:
* You will conduct regular reviews of funding and client/carers satisfaction to ensure our services are meeting the required standards.
* Consumer Support Plans:
* The successful candidate will complete consumer support plans and assessments within the required timeframes, ensuring seamless delivery of services.
* Case Conferences:
* You will facilitate case conferences with internal and external stakeholders in conjunction with the consumer and/or their representative when needs change or review of needs is required.



Requirements

* Demonstrated Experience: In a Community Services Co-ordination or Case Management role
* Qualifications: Certificate IV in Community Services (Preferred)
* Skills: Strong written and verbal communication skills; Ability to work effectively as part of a multidisciplinary care team; Knowledge of OH&S legislation relevant to aged care and disability service sector
* Additional Requirements: NDIS Worker Screening Check; National Police Certificate; Current Driver's License
