We are launching our first dedicated Community Aged Care Physiotherapist role — and we’re looking for someone who genuinely believes in helping older Australians live well at home.
This role is exclusively supporting older clients under the Support at Home / Home Care Package program.
You’ll work 1:1 with older clients in their own homes — supporting mobility, confidence, independence and overall wellbeing.
About the Role
* Deliver in-home physiotherapy to support mobility, strength, balance and pain management
* Conduct functional assessments and develop tailored care plans aligned to individual goals
* Provide falls prevention, education and strength-based interventions
* Apply a holistic, wellness-focused approach to promote healthy ageing
* Collaborate with families and other stakeholders
* Maintain accurate and compliant clinical documentation
* Manage your own caseload with support from our multi-disciplinary team
* Travel locally within your region
You’ll enjoy genuine flexibility, the ability to complete administration from home, and the support of an experienced multidisciplinary team and award winning National Manager.
What We’re Looking For
* Experience in Community Aged Care or Home Care Package environments
* Confidence working independently and managing your own schedule
* Communicates with warmth, clarity and professionalism
* Values autonomy but enjoys being part of a connected national multi-disciplinary team of OTs, Speechies, Psychologists and Behaviour Support Practitioners.
* You must hold current and full AHPRA registration, a reliable vehicle and valid driver’s licence.
