Healthcare Coordinator Role

We are seeking a skilled and dedicated individual to join our team as a Healthcare Coordinator. In this role, you will play a vital part in ensuring the delivery of high-quality healthcare services to clients with complex care needs.

Your primary responsibility will be to develop and implement client care plans that meet the unique needs of each individual. This will involve working closely with clients, their families, and our support staff to ensure that care is tailored to meet each person's specific requirements.

As a Healthcare Coordinator, you will also be responsible for providing clinical guidance and support to our staff, helping them to deliver exceptional care to our clients. You will work closely with our clinical team to ensure that all aspects of care are coordinated and delivered in a timely and effective manner.

Key Responsibilities:

* Develop and implement client care plans that meet the unique needs of each individual
* Provide clinical guidance and support to our staff
* Work closely with our clinical team to ensure coordination and delivery of care
* Identify and assess clinical matters, obstacles, and barriers impacting client care
* Facilitate inductions, education, and training workshops for support staff
* Conduct clinical competency assessments for support staff
* Manage stock orders for clients
* Review client progress notes
* Manage clinical compliance for all clients to meet Attendant Care Industry and National Disability standards
* Liaise with allied health, support coordinators, and other external stakeholders
* Visit clients regularly and maintain impactful relationships

Requirements:

* Current AHPRA registration as a Registered Nurse (with nil restrictions)
* Passion for health education, demonstrating skills in health assessment, planning, and goal setting
* Strong communication skills (both oral and written), organisational, and computer skills
* Ability to work remotely and autonomously when required
* Understanding of NDIS/TAC and other funding bodies (desirable)
