Job Description

We're now looking for a people-focused and operationally strong Team Leader, Client Transition to join our PMO team and play a key role in delivering an important transformation program that supports positive outcomes for our clients and provider partners. This is a full-time, maximum-term role to 31/03/2027.

About the Role

Reporting to the PMO Manager, the Team Leader, Client Transition will lead a dedicated team responsible for coordinating the safe and seamless transition of client services as a part of Care Connect's strategic project.

This role is critical in ensuring clients experience continuity of care, clear communication and minimal disruption throughout the transition process.

You'll provide leadership, operational oversight and stakeholder coordination while supporting successful project delivery in a fast-paced and evolving environment.

What you'll be doing

* Leading and supporting a team of Client Transition Coordinators
* Coordinating end-to-end client transition activities across internal teams and external providers
* Monitoring transition schedules, risks, milestones and performance targets
* Managing escalated client matters and supporting positive client outcomes
* Building strong relationships with operational stakeholders and provider partners
* Preparing reporting, maintaining accurate records and supporting governance activities
* Identifying opportunities for continuous improvement across transition processes
* Promoting a collaborative, accountable and client-centred team culture
About You

You are an experienced people leader who thrives in operational environments where communication, coordination and client experience are key.

To be successful, you will bring:

* Experience leading or coordinating teams within aged care, home care, community services or similar operational environments
* Experience managing client transitions, service coordination or operational delivery activities
* Strong stakeholder engagement and relationship management skills
* Excellent communication, organisation and problem-solving capabilities
* Experience managing escalated client concerns or complaints
* The ability to work effectively in fast-paced and changing environments
* Strong reporting and administrative skills, including Microsoft Office proficiency
* A client-first mindset with a commitment to quality outcomes

Qualifications in business, community services, health administration, project management or a related discipline will be highly regarded.

Occasional regional or interstate travel, including overnight stays, may be required.

To be considered for this role you must have full Australian working rights, current Police & Working with Children Checks – we can arrange police checks for successful candidates. Our selection process also includes psychometric assessment.
