We are seeking an experienced Registered Nurse to join our Northgate clinic team, providing frontline nursing duties and contributing to the organisation's comprehensive health support services for improved client outcomes.
The successful candidate will deliver education to clients regarding their care, promote opportunities for enhanced health outcomes, and adhere to the IUIH Model of Care.
Main Responsibilities:
* Providing direct patient care according to the IUIH Model of Care
* Conducting health assessments, developing care plans, and coordinating interventions
* Educating clients on healthy practices and promoting self-management
