THE ROLE
The Disability Practice Advisor supports the care of children with complex disability who require additional mainstream supports to sustain their care in the family home, or who are residing outside the family home. This includes supporting NDIS registered providers delivering services to children with a complex disability and their families and implementing board and lodging arrangements for children living outside the family home. The role also works collaboratively with Child Protection and Family Services, providing advice in relation to disability and NDIS navigation, to improve outcomes for children with a disability and prevent family breakdown.

ACCOUNTABILITIES
* Support the response to children and young people with a disability at risk of or requiring an alternative care placement, including early intervention and family preservation support, working with service providers and contributing to subsequent care arrangement decisions when determined to be in the best interests of the child or young person.
* Provide planning for children and young people who are unable to live in the family home due to their complex disability needs, including monitoring and oversight of the board and lodging arrangements in accordance with relevant policy and operating frameworks for children and young people with a disability.
* Participate in case coordination and support according to individual circumstances to ensure children and young people in care arrangements can either return to living with their families or smoothly transition into adult services which meet their long‐term needs.
