Key Responsibilities
* Deliver safe, evidence-based nursing care to clients in their homes
* Conduct health assessments, care planning, and ongoing reviews
* Diabetes management, continence care, and other clinical procedures
* Monitor and evaluate client health status and escalate concerns when required
* Collaborate with multidisciplinary teams, families, and external health services
* Maintain accurate documentation in line with organisational and regulatory requirements
* Promote client independence and support positive health outcomes
* Provide education to clients and carers regarding health conditions, medications, and self-management
* Comply with all clinical, safety, and organisational policies and procedures
