Home Care Professional

About the Role

The Care Advisory Partner (CAP) develops strong relationships with clients to understand their needs and preferences. They create tailored service plans, assess clients, onboard new clients efficiently, and provide handovers to the operations team. The CAP collaborates with internal and external stakeholders to ensure a seamless process for clients to access home care services.

This role offers a 12-month fixed term contract with hybrid opportunities available. You may work out of various locations, including St Leonards, Hammondville, Tuggerah, Newcastle or Nowra. This role is available full-time or part-time, with workload aligned to days worked.


Key Responsibilities

* Client Engagement: Promote home care services to potential clients, community organisations, and referral networks; deliver clear information to clients so they understand available support options and eligibility criteria.
* Referral and Service Coordination: Build strong referral pathways with external stakeholders, liaise with internal teams and external providers to streamline access to services, and facilitate timely linkage of clients to other home care services.
* Client Assessment and Intake: Review external assessments to inform care planning and service eligibility; conduct holistic assessments of clients' care needs and develop personalised care plans.
* Service Agreement and Onboarding: Lead discussions with clients to confirm understanding of service options, responsibilities, and financial implications; finalise client agreements and initiate service setup.
* Care Transition and Handover: Ensure smooth handover of ongoing care to the designated Care Manager once services commence.


About You / Qualifications

* Commitment to align with our organisation's values and mission.
* At least one year's experience in home care/community as a Care Manager or in an Admissions function preferred.
* Experience bringing new clients onto Homecare packages preferred.
* Extensive skills and experience in undertaking comprehensive assessments in an aged care community setting.
* Demonstrated knowledge of relevant legislation, regulations, and industry standards.
* A current driver's licence, reliable car, and safe driving record. Willingness to travel.
* Formal qualifications in a care related discipline, preferably Diploma or Degree level.
* Willingness to undertake and complete training requirements.
