Job Description: We are seeking a highly skilled and compassionate female preferred NDIS Support Worker with experience in supporting individuals and families with cognitive impairment and end of life family care, 3 days per week. The successful candidate will play a crucial role in providing person-centered support to our participants, promoting their independence, well-being, and quality of life. Key Responsibilities: Providing direct support to individuals and families with complex care needs. Implementing tailored support plans designed to meet the unique needs and goals of each client. Managing challenging behaviors, including aggression and self-harm, in a safe and effective manner. Assisting clients with daily living activities, personal care, and community participation. Building positive relationships with clients, their families, and other support networks and understanding end of life care processes. Documenting client progress, interventions, and incidents in accordance with NDIS guidelines. Collaborating with multidisciplinary teams and external service providers to ensure holistic care and support. Transporting using own vehicle to appointments. Requirements: Minimum of two years of experience working as an NDIS Support Worker, with a focus on cognition and complex care needs. Demonstrated understanding and experience in supporting individuals with autism and non-verbal communication. Proven ability to manage challenging behaviors, including aggression and self-harm, in a calm and professional manner. Familiarity with NDIS documentation requirements, including note writing and reporting. Certificate III or IV in Disability, Individual Support, or a related field. Current First Aid and CPR certification. Excellent communication and interpersonal skills, with the ability to build rapport with clients and their support networks. Strong problem-solving skills and the ability to adapt to changing situations. A compassionate and person-centered approach to care and support.
