To find out more about the Services at Country Health Connect - Flinders and Upper North Community Health Service | SA Health Living and Working in the Flinders and Upper North Region - Port Augusta About the Role: As the Associate Nurse/Midwife Unit Manager - TCP Coordinator, you will provide high-quality nursing and/or midwifery services while supporting the leadership and coordination of the ward, unit or service. Building on your experience and professional development, you will practise at a proficient level, accepting accountability for your own standards of care, delegated activities and the guidance of less experienced staff. In this role, you will work collaboratively with the Nurse/Midwife Unit Manager and fellow AN/MUMs to promote continuity and consistency of care, assist with practice improvements and contribute to unit management responsibilities such as staffing, rostering, performance management and resource planning. You will also coordinate and deliver services associated with packaged care programs, including the Transitional Care Program (TCP), applying Primary Health Care principles to support service planning, implementation and evaluation in partnership with a diverse range of stakeholders. About You: Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate. Registered Nurse/Midwife with at least 3 years post registration experience or currently classified as a Clinical Nurse/Midwife. Demonstrated competence in the relevant area of nursing practice in accordance with the appropriate standards of practice. Experience in the supervision of student nurses, enrolled nurses and less experienced registered nurses.
